ITEM: PEDIATRIC X-RAY UNIT (QTY: 1)
DESCRIPTION: This stationary X-ray unit to be used for pediatric patients to provide for
rapid diagnosis with decreased exposure to harmful radiation.
ITEM: MOBILE PEDIATRIC X-RAY UNIT (QTY: 1)
DESCRIPTION: This mobile X-ray unit to be used for pediatric patients to provide for rapid diagnosis with decreased exposure to harmful radiation. This unit will be used on patients that are immobile or patients that are in the intensive care unit or operating room.
ITEM: MOBILE C-ARM UNIT (QTY: 1)
DESCRIPTION: This mobile C-Arm unit to be used in the Pediatric Endoscopy Suite.
ITEM: ENDOSCOPIC MULTI-MEDIA SYSTEM (QTY: 1)
DESCRITION: This system should include the power light source, the central processor, picture-capturing component and central monitor.
ITEM: ENDOSCOPIC WASHING MACHINES (QTY: 2)
DESCRIPTION: These machines will be use to clean/disinfect Endoscopes. The machine must be equipped with dual chambers.

The DEADLINE FOR SUBMISSION OF QUOTES is 45 days from the posting of this bulletin.
All manufacturer warranties on proposed equipment must be honored in Israel. The quotation must contain the name of a reliable Israeli agent that will carry out the terms of the warranty and who can be used for installation (if needed), spare parts and other services. Offers are to be in compliance with USAID source and nationality requirements enumerated in 22CFR, part 228. In order for a quote to be responsive to this notice, suppliers are required to provide a copy of the applicable approval letter from FDA, covering medical devices that require pre-market in the USA.
Additionally, the supplier shall provide the following certification: “The undersigned supplier agrees and certifies that all products provided are covered by a valid FDA approval letter and/or otherwise are in compliance on the date of shipment with all applicable United States of America laws and regulations governing the delivery of regulated products in interstate commerce”
