Sealed bids to provide the following goods will be received by the Hadassah Medical Organization Jerusalem until 4 pm Israeli Time on Monday 4th  of December  2017. All bids will be opened and reviewed at a time of Hadassah’s choosing thereafter.

 

Item: Neuro Angiography Room  (Cath lab) QTY: 1

Description: This state-of-the-art Neuro Angiography Dual Plane Room will be used for Angiographic Examinations of the Head and Neck

A purchase order contract is intended to be awarded to the lowest responsive responsible bidder on a firm fixed price basis. The proposed contract document is an addendum to this solicitation. Bidders are encouraged to thoroughly examine the terms and conditions in the proposed contract before submitting a bid. Funds to pay the cost of this procurement have been made available by the United States Agency for International Development (USAID) which requires that certain laws and regulations referenced in the contract apply to the transaction (2 CFR 200.326 and 2 CFR 200, appendix II as applicable). Hadassah reserves the right to waive technicalities or defects or to reject any and all bids. All bids received and accepted shall be binding for a period of 180 days after the deadline identified above. A bidder may withdraw a submitted bid prior to the deadline date identified above in which case that bid will not be considered.  Any questions concerning the above description or specifications shall be submitted to Anita Rotenberg. Hadassah may make such inquiries as it deems necessary to determine the ability of any bidder to provide the items sought including the conduct of vetting against data bases maintained by the United States Government whose funds are being used by Hadassah to make the purchase.

Please specify the equipment requested. Upon receipt of your request, a copy of the solicitation will be sent to you by email.

All manufacturers' warranties on proposed equipment must be honored in Israel.  The quotation must contain the name of a reliable Israeli agent that can carry out the terms of the warranty and who can be used for installation (if needed), spare parts and other services.

In order for a quote to be responsive to this notice, suppliers are required to provide a copy of the applicable approval letter from FDA, covering medical devices that require pre-market approval in the US and the Israeli equivalent of FDA approval which is known as AMAR

Additionally, the supplier shall provide the following certification:

“The undersigned supplier agrees and certifies that all products provided are covered by a valid FDA approval letter and/or otherwise are in compliance on the date of shipment with all applicable United States laws and regulations governing the delivery of regulated products in interstate commerce.”
